Ummm google forms? It’s free and will dump into a google sheet. (Haven’t looked to see if I can see what your form was to know if I’m making a dumb recommendation or not.)

Ooh that would be nice, since I already use google sheets. I want to embed it nicely in my website - can google forms do that?

I can’t remember that exact detail. Give me a min to look on one that I already have.

Send > then choose send via <> and you have some embed html to pop into a webpage.

It's an iframe by the way. (I picked an unfinished junky form to test on so I don't want to accidentally share that mess.🤣 )
